However, many patients with chronic conditions such as hypertension, diabetes, or cardiovascular disease often hesitate to pursue implants or abandon the idea altogether due to concerns about their health.
In the past, these conditions were often viewed as near-absolute contraindications for implant therapy. But recent advancements in medical technology and precise preoperative assessments have made implants a viable option for most patients with systemic diseases, provided there is thorough evaluation and a tailored approach.
Dr. Keun-Il Kim, chief dentist at 365 Seoul One Top Dental Clinic, explains, “For diabetic patients, poorly controlled blood glucose levels can delay wound healing and increase infection risks, potentially raising implant failure rates. However, studies now consistently show that diabetic patients with stable blood sugar levels can achieve implant success rates comparable to those of healthy individuals. With diligent blood glucose management before and after treatment, combined with antibiotic prescriptions and rigorous hygiene protocols, favorable outcomes are entirely achievable.”

Similarly, patients with hypertension or cardiovascular conditions are benefiting from new approaches. “In the past, concerns about bleeding or sudden blood pressure spikes led many to avoid implants,” Dr. Kim notes. “Today, minimally invasive techniques like flapless digital-guided surgery and conscious sedation methods help minimize blood pressure fluctuations while reducing patient stress.”
Dr. Kim also highlights the role of advanced diagnostics in improving outcomes. “The introduction of CT-based 3D digital analysis allows us to precisely assess bone density and anatomical structures. This has led to shorter surgical times and reduced bleeding, which is particularly critical for elderly patients or those with chronic conditions.”
Not every patient with systemic disease is a candidate for implants, however. Severe heart failure, uncontrolled hypertension, active infections, or bleeding disorders may warrant postponing treatment, as determined by medical professionals. Comprehensive medical history reviews, detailed consultations, and collaboration with primary care physicians are essential prerequisites.
Post-treatment care is equally critical, especially for patients with chronic conditions. Regular check-ups, meticulous oral hygiene, and lifestyle adjustments related to their systemic diseases are vital for long-term implant success.
Dr. Kim emphasizes that chronic illness should not automatically rule out dental implants. “Modern dentistry, equipped with digital diagnostics and safe surgical systems, allows most patients with chronic conditions to undergo treatment with a customized plan. The key lies in thorough consultations with medical professionals and diligent post-treatment care,” he advises.
